Well Connected is a programme operating across Lanarkshire, sometimes referred to as social prescribing or community referral. It helps people access activities, groups and services that can improve mental health and wellbeing.
The programme aims to make it easier for individuals to become involved in opportunities within their local community and benefit from the support available. All services are provided free of charge.
